Med-IC is actively engaged in a pilot project to identify and recruit a small number of hospitals or health systems that have demonstrated readiness to develop a suite of innovative geriatric care models. This "service line" or "portfolio" model would target mid-size hospitals that suffer economic disadvantages from low-profit margin Medicare medical cases and would help keep older people out of the hospital who do not need to be admitted, care effectively and efficiently for those patients who do need to be admitted, and transition patients out of the hospital effectively and prevent hospital readmissions. This portfolio will be fiscally attractive to potential customers and will result in improvements in the quality of care provided to patients.
